A randomized, double-blind study has been conducted in which 87 patients between 8 and 40 years of age who had newly diagnosed type 1 diabetes were assigned to receive infusions of Rituximab or placebo on days 1, 8, 15, and 22 of the study. The primary outcome, assessed 1 year after the first infusion, was the geometric mean area under the curve (AUC) for the serum C-peptide level during the first 2 hours of a mixed-meal tolerance test. Secondary outcomes included safety and changes in the glycated hemoglobin level and insulin dose.At 1 year, the mean AUC for the level of C peptide was significantly higher in the Rituximab group than in the placebo group.A four-dose course of Rituximab partially preserved beta-cell function over a period of 1 year in patients with type 1 diabetes.
Residual beta-cell function results in better glycemic control, less microvascular complications and less hypoglycemia. And consequently, marked clinical benefit might result from preserving the higher levels of C-peptide observed in patients with surveillance versus community diagnosis of type 1 diabetes. In addition C-peptide itself may have biologic effects, most of which are proposed to be beneficial.

Parsing I think what is giving you the problem here is you are parsing it wrongly: it should be 'surveillance (diagnosis)' / 'community diagnosis', as SJLD has kindly pointed out.

One of the first points to make is that the diagnosis of type 1 diabetes is dramatically different when made in the community, almost always due to presentations with clinical signs and symptoms of marked hyperglycemia, versus when made as part of a surveillance program of high risk non-diabetic subjects.

Hi Tony, 1) right, it is diagnosis within the community 2) "diagnostic de communauté" is actually mentionned in my 1st ref. 3) my 2nd ref. is about health research in a community in North Carolina.
