This is under the section "Osjećaj nemira".
I realize that it means "I break my arms" literally, but obviously this has some idiomatic meaning that I'm not familiar with.

Yes, it means the same thing. It is listed in the Bujas' 2001. edition on page 317, on the bottom of the right column, under 'ruke'.

It does have an idiomatic meaning (to despair, lament), but in this particular case the phrase is more likely to have the physical meaning of bending or curling one's arms (not of breaking them) at the elbows - possibly, while waving them about - as a sign of anxiety or nervousness. This explanation is also suggested by the mention of the other two symptoms: fidgeting and not being able to sit still. Somehow, the figurative meaning doesn't fit in there.
