Solid Converter and Convert Doc seem to do the job, but both have small issues with pages and tables.
I also tried Adobe Acrobat Professional 8, but it seems like these two programs (Solid Converter and Convert Doc) work better. The word document exported from Acrobat looks very bad.
I heard Abby Finereader would do a good job. Has anyone tried it?

I tried Abby Finereader.
Is good for text but when you have images and tables I don't recommend it.

With tables and images, it is a bad idea to let FineReader recognise them automatically. However, if you tell the program what areas are images and tables (i.e., you define them manually), the quality of output is much improved.
